___ I understand that Chorus membership is an honor that my child must work to maintain; if my child earns D, F, or 3 report card grades or develops a pattern of inappropriate behavior in any area at PPE, (s)he may be suspended from choir;
___ I have read and understand the Plantation Park Singers Chorus Handbook and rehearsal/ performance schedule;
___ Attendance and good behavior at ALL rehearsals and performances is MANDATORY; absences are excused in cases of illness, death in the family, occasional lack of transportation, or prearranged family event ONLY. Non-emergency doctor/dentist appointments must be scheduled on non-chorus days;
___ THREE (3) UNEXCUSED ABSENCES WILL RESULT IN REMOVAL FROM CHORUS. Five or more excused absences MAY result in suspension of performance privileges. Parents must turn in written notes to Mrs. Greggs regarding chorus absences within 2 rehearsals of each absence;
___ Our choir’s performance excellence, and each child’s end-of-the-year awards and participation in field trips depend on strong attendance and good behavior (see point system outlined in Chorus Handbook);
___ For the protection of all involved, this contract is legally binding. I agree that my child will attend rehearsals for one entire school year, and the school agrees to provide supervision for my child during established rehearsal times. This contract may be broken only by written notification between the school and the parent/guardian;
___ I agree to pick up my child promptly at dismissal time. If my child is picked up late (more than 10 minutes after dismissal) 3 times, (s)he may be removed from chorus;
___ If my child can not continue in chorus for whatever reason, I will return the chorus folder and any borrowed uniform items to Mrs. Greggs as soon as possible. If I do not return these items, I will pay a $40 textbook fee before my child is permitted to move on to middle school;
___ I give my child permission to be recorded/photographed as part of the Plantation Park Singers;
___ I agree to do my best to donate a minimum of 2 hours volunteer time (at home or at school) to Plantation Park Singers this year in the following areas (please circle at least two):

¯ This form and all Chorus Package materials (contract and $20 Registration Donation* (CASH ONLY) must be completed in full no later than the first rehearsal.
